Adolescent Family Life Program (AFLP)  Printer Friendly View

The Adolescent Family Life Program (AFLP) is a voluntary multidisciplinary case management team for pregnant and/or parenting teens that are under age 19 at the time of inital enrollment.  Case management services can be provided for teen mothers until age 20 years and fathers of babies are eligible for the program until the age of 22.  Exceptions to the age limitations can be made by the case managers on a case by case basis.

Goals:
Reduce the number of low-birth weight babies and improve pregnancy outcomes.
• Prevent or delay subsequent pregnancies.
• Maintain teen parents in school or facilitate re-entry of school dropouts.
• Improve infant health.
• Reduce the incidence of child abuse/neglect through the development of good parenting skills.

Eligibility: 
Women under the age of 19 who:
      • Have a confirmed pregnancy and have made a decision to continue their pregnancy either to parent or to place the infant for adoption.
      • Are parenting one or more children.
Men under the age of 22 who:
      • Are co-parenting with a pregnant or parenting adolescent woman enrolled in AFLP.
      • Must be actively involved in caring for their child.

Services: 
• Comprehensive assessment including domestic violence and substance abuse screening.
• Establishment of an individual service plan (ISP).
• Referrals for medical care, job training, education, financial assistance, legal advice, counseling, food, housing, parenting classes, childbirth preparation, family planning and child care.
• Advocacy.
• On-going case monitoring by case manager through monthly home and office visitation.
