Child Health and Disability Prevention Program (CHDP)  Printer Friendly View

CHDP Program Activities

The Child Health and Disability Prevention Program (CHDP) is a preventive health program serving San Mateo County's children and youth. CHDP assures that early and periodic health care is available to all eligible children, not only children with obvious health problems.

Through the CHDP Program, children and youth can obtain regular preventive health assessment to identify any health problems. Those with suspected health problems are then referred for necessary diagnosis and treatment. Many problems can be prevented, corrected, or have the severity reduced by prompt diagnosis and treatment. The earlier health problems are found, the more easily they can be treated & corrected.

CHDP coordinates efforts with a wide array of persons and organizations to insure that eligible children receive needed care: private physicians, health departments, schools, nurse practitioners, dentists, health educators, nutritionists, welfare and community and managed care organizations.

Your child should have health checks at the ages listed below. CHDP will pay if your child is eligible.

Who is Eligible for CHDP Preventive Health Services?

  • Medi-Cal families. All Medi-Cal recipients from birth to age 21
  • Income-eligible youth from birth to age 19
  • Children who attend Head Start or state preschools
  • Children in foster care
CHDP Health Assessments Include
  • Health and developmental history
  • Head-to-toe physical exam
  • Nutritional assessment
  • Dental screening
  • Immunizations
  • Vision testing and hearing testing
  • Lead testing
  • Some lab tests (e.g., tuberculin, sickle cell, urinalysis, hemoglobin/hematocrit, Pap smears)
  • Health education and anticipatory guidance
Getting A CHDP Exam
  1. Call the San Mateo County CHDP office at (650) 573-2877 to confirm eligibility.
  2. If you are eligible, ask if your regular doctor or clinic can give you a CHDP exam. If they cannot, get the name and number of a CHDP doctor.
  3. Call your doctor or clinic and ask for an appointment for a CHDP exam.

Exams are provided by CHDP approved providers located throughout the County.

CHDP encourages private provider participation. San Mateo County assists providers with patient tracking and ensuring that families make and keep their appointments.
